Medicaid Waiver Programs
Medicaid Waiver Programs are a set of programs offered by the United States government’s Medicaid program. These programs aim to provide home and community-based services to individuals who would otherwise require institutional care. Two specific programs within Medicaid Waiver Programs are the Nursing Home Transition Diversion (NHTD) and Traumatic Brain Injury (TBI) waivers.
The Nursing Home Transition Diversion (NHTD) waiver helps individuals transition from nursing homes to their homes or community-based settings. It offers support services like personal care assistance and home modifications, promoting independence.
The Traumatic Brain Injury (TBI) waiver provides services to individuals with traumatic brain injuries. Rehabilitation, assistive technology, and behavioral support are offered to help them regain independence and improve their quality of life.

To qualify for the NHTD and TBI Medicaid Waiver Programs, individuals must meet specific criteria such as being eligible for community-based Medicaid, requiring nursing home level of care, and being able to live safely in the community. Eligibility
Qualifying for the NHTD and TBI Medicare Waiver Programs
While at Care Connection we cannot make or assist with the application, we are a participating LHCSA. We can direct the applicant to the Regional Resource Development Center (RRDC), the Suffolk Independent Living Organization (SILO) so they can begin the process of qualifying. The qualifying factors are as follows:
NHTD Medicaid Waiver Program
• Eligible for community-based Medicaid
• Need for nursing home level of care
• Age: 18-64 years with a verified physical disability or 65 years or older
• Ability to live safely in the community
TBI Medicaid Waiver Program
• Eligible for community-based Medicaid
• Need for nursing home level of care
• No age restriction (applies to individuals of all ages)
• Ability to live safely in the community
